What is the registration procedure with United FC?
1. The 1st step in the process is placement on a team. This notice will come directly from your team Coach.
2. If you are a returning player and were provided a Statement of Commitment, you must sign the letter and make a deposit as instructed. The payment and the signed letter should be given to the Team Manager.
3. When instructed by your Team Manager, go to the United FC Registration site.
a. If you are a returning player but don’t remember you account username & password, click here to retrieve your information. If that does not work please contact your team manager.
b. Once logged in, complete the information as required. We would greatly appreciate your payment by credit card. To encourage this, we are not charging a convenience fee for credit cards. Rack up those rewards points!
c. Lastly, remember to:
i. Print Player Registration Complete (4 of 4) page
ii. Print and sign 3 copies of the CYSA-S Medical Release Form.
4. Lastly, submit to your Team manager the following:
a. Signed CYSA-S Medical Release (3 copies)
b. If you did NOT have a CYSA-S Player Card issued to you by United FC for the 2013/14 season then please also provide an original proof of age document (birth certificate or passport) along with 3 copies. The original will be returned to you.
c. Printed copy of the Player Registration Complete page (Step 4.c.i. above).
d. If you choose not to pay by credit card, a check / money order / cash payment of the deposit is required.
